Every hospital CISO knows the authentication problem. Fewer recognize that the mental model of "biometric" they've been working from is part of what's holding them back.
When most people hear "biometric authentication," a specific image comes to mind. A fingerprint reader on a laptop, a face scan at an airport, or a retina scanner in a spy movie. In every case, the pattern is the same: you present something about your body, a sensor reads it, and the system decides whether to let you in.
This mental model has a few things baked in. First, biometrics require a deliberate action from the user. You have to place your finger, look at the camera, or stop and complete whatever action the sensor requires. Second, they happen at a specific moment: login. The biometric authentication takes place, the door opens, and then the system stops asking who you are. Third, they depend on hardware: a reader, a camera, or a sensor that has to be procured, maintained, and present at every access point.
That's the model. And it's worth noting, because behavioral biometrics doesn't fit it at all.
Under the NIST authentication framework, behavioral biometrics is classified as a "something you are" factor, the same category as a fingerprint or a face scan. The classification is correct. The experience is completely different.
Where traditional biometrics verify who logged in, behavioral biometrics verifies who is still at the keyboard.
That's not a subtle distinction. It's the difference between a locked front door and someone actually paying attention to what's happening inside.
In almost every conversation with health systems, the same question surfaces: "Is it as accurate as a fingerprint?"
It's a fair question, and the answer is yes. Twosense's origins are in the Department of Defense, where accuracy requirements are not a suggestion. But accuracy is actually the less interesting part of the answer. The more important point is that behavioral authentication catches something a fingerprint simply cannot.
A fingerprint verifies who unlocked the device, and then it's done. An authorized user badges in, authenticates cleanly, and walks away from an unlocked shared workstation. From the fingerprint reader's perspective, everything is fine. From a security standpoint, the session is now wide open to anyone who walks up to the shared workstation next.
Behavioral authentication continues asking "is this the authorized user?" every second for the entire session. When the authenticated user walks away and someone else steps up to the machine, the system flags the behavioral mismatch and forces the new user to reauthenticate as themselves. The fingerprint reader clocks out at login. Behavioral authentication never clocks out.
Trust models learn continuously. As the user works, machine learning updates their behavioral profile in the background. Say someone returns from an injury and is now typing with one hand. The pattern has changed significantly. The system detects the change and triggers step-up authentication. The user verifies themselves through a secondary method, and that verification tells the system: this is still the authorized user, learn this pattern.
Over the following days, as the user continues working with their altered behavior, the model absorbs the new pattern and adjusts. The same happens in reverse when the original behavior returns. The system adapts because it was designed to. A fingerprint reader, by contrast, cannot adapt. A burned fingertip or a callus from a new hobby is just a failed scan.
Behavioral biometrics is not a snapshot of who you are. It's a living, updating model of how you work.
In a clinical environment, authentication friction is not an abstract inconvenience. It is a daily operational cost measured in time, interruptions, and workarounds that create the very security gaps the friction was meant to prevent.
Hospitals implement 15-character complex-password policies that staff hate. Shared workstations time out mid-task. Clinicians share credentials because the alternative is re-authenticating every time they switch rooms or terminals. Badge tap solutions help, but they still require a deliberate action and a piece of hardware at every endpoint. The result is an authentication experience that is either secure or fast, but rarely both.
Behavioral authentication eliminates this entirely. The clinician badges in, and from that point forward, authentication is happening invisibly and continuously, in the background. There is no prompt or additional step introducing friction. When someone else sits down at an unlocked workstation, the system detects the behavioral change and allows IT to implement remediation based on the associated policy: trigger a step-up, sign out the session, lock the endpoint. The authorized user never noticed the authentication. The unauthorized user is flagged, and the hijacked session terminates.
This is already running at scale. A leading U.S. health system has deployed behavioral authentication across 17,000 users and 173 applications. It continuously verifies identity without adding a single authentication step to clinical workflows.
What makes something a biometric is not that it scans you. It's that it uses something unique to you to verify who you are. By that definition, behavioral patterns are among the most powerful biometrics available.
